Sign-up Fees:
$105.00 for age groups U6 to U8
$115.00 for age groups U10 to U14 (please note - there will be no U16 and up age groups for Spring season)

1”X 1-1/2” color photo of players head (no hats or sunglasses)
Note: Picture must fit in the box above and to the right, and face must be clearly seen.
* Birth Certificate Requirement for ALL Players
A copy of players birth certificate or passport is required for ALL PLAYERS
* Two Filled Copies Required Of The Registration Form and One Copy Of The Code of Conduct Signed
Carbon registration forms will be provided at location along with the code of conduct form.
You may download these forms at to the link on the left side.
* YOU MUST BRING (2) FILLED OUT COPIES (otherwise, we will ask you to re-fill out a new one - official forms will also be provided at location)
NOTE: No application can be accepted without a picture of proper size, (passport pictures are too big, a snapshot of proper size is fine), birth certificate or passport, carbon registration form filled out and signed, and code of conduct signed. Application will only be accepted by parent or
guardian.
Volunteer to Coach!!
MHYSL is always looking for coaching staff to work with our youth soccer players. It is our volunteer parents within our community where it all begins and inevitably blossoms our youth soccer players into great athletes. As coaches, you will not only be recognized for your gracious contributions, but will also gain the satisfaction of knowing you have helped the youth of our community. To become a coach, simply mark the box on your registration form that you are in fact committed to coaching. MHYSL will help you get started with training and on your way to a successful and fun soccer season!
